10. Medical data (Tattoo Health Questionnaire)
Before each tattoo session, we ask clients to fill out a short health questionnaire, where we may collect special categories of personal data, including:
- Allergies (e.g. to latex, disinfectants)
- Skin conditions or infections
- HIV/AIDS or other bloodborne diseases
- Medications that affect healing or bleeding
- Other health-related notes
This data is collected in person, in paper or electronic form during the visit to the studio. It is not stored on the website.
We process this data solely to ensure the health safety of the client and hygiene during the tattooing process.
Legal basis:
- Article 9 paragraph 2 point a of the GDPR – Explicit consent of the client (given by signing the form)
- Art. 6 para. 1 point d GDPR – Protection of vital interests of the data subject
Only the tattoo artist performing the service and the studio management have access to the data. The data is stored securely and is not shared with third parties.
Storage period: 3-5 years, in accordance with health regulations and legal provisions.
